The Federal Motor Carrier Safety Administration requires that interstate commercial motor vehicle drivers maintain a current Medical Examiner’s Certificate to drive. As part of the physical, you are required to fill out the medical history portion of the Medical Examination Report of Commercial Driver Fitness Determination.

When you go to the DMV to get your CDL, you’ll be expected to show either your DOT medical card, or the DOT physical long form. Additionally, any employer hiring you as a CMV operator is going to require you to present your DOT physical card, and possibly the long form physical, as well.
 Why is the DOT physical examination important?
The USDOT physical examination is required to help ensure that a person is medically qualified to safely operate a commercial motor vehicle. In the interest of public safety, commercial motor vehicle drivers are generally held to higher physical, mental and emotional standards than passenger car drivers.
